Dyslexia is“any impairment of the ability to read”; it is a general term, describing the effect of the disability, not its causes, which may be many and varied. 



Dyslexic is the adjective, as in He has some sort of dyslexic problem not yet really diagnosed, and it also serves as the noun meaning“one who has symptoms of dyslexia,”as in His teachers think he's a dyslexic.
